The forms listed are provided as a courtesy. This list is a concise list. For a full catalog of forms, please visit the Indiana Secretary of State's page under the Elections tab. You will need to sign the forms in front of a Notary Public. If you are unable to find a notary then the Clerk's office or Election Division may be able to assist you during normal business hours Monday through Friday from 8:30am to 4:30pm.
